EyeMed is a vision insurance (vision carrier/vision rider) that is added on to a patient's medical insurance.
Log on to the EyeMed website to check the patient's benefits, eligibility, copay and to take an authorization. We need to claim this authorization on the day of the patient's exam. Please copy the authorization number into the exam notes on TAB with copay information.
Always take a copy of the patient's medical insurance card, even if they are using EyeMed benefits for the routine exam. We do not need a copy of the EyeMed card.
