Pre-appoint Patient and Set Communication Preference: "The Doctor recommends you have an annual eye exam. I am going to make that appointment for you now. Would you like us to contact you next year by phone, email, or text?". Choose "Yes" for their preference + leave mail as "Yes".
Hand the prescription and receipt to the patient.
Offer to make eye exam appointment for any friends or family members.
Check Eyemed eligiblity for patient's family while they are in the exam room. Offer to make exams for those still eligible.
Schedule follow-up appointments.
If scheduling a contact lens follow-up appointment, remind patient to wear their contact lenses when they return, and inform them of our Contact Lens Release policy.
If scheduling a medical follow-up (Office Visit), check if patient needs a referral (HMO plans do). If so, tell patient they need referral before they can be seen and make a note on the follow-up appointment on TAB that patient was told they need a referral. PCP should make referral in name of Dr. Anthony Fusco and fax to the office fax number that is on the appointment reminder card.
Be sure to scan any forms from the Doctor before giving to the patient.
BEST PRACTICES:
Review doctors notes on Patient Info Form for when patient should Return to Office.
Schedule follow-ups and offer to write on business card for patient.
Ask if they would like to make appointments for additional family members. Offer to check eligibility of other members for patients with Eyemed.
If patient is returning for CL follow-up, remind them to wear their contact lenses to the appointment.
If patient is returning for Office Visit, verify if they need a referral. If so, educate patient and make note you told them in next appointment notes.
