Understanding Changes to SNAP, Medicaid and Medicare with the Unwinding of the additional Pandemic Funds
SNAP Benefits:
If you are currently receiving SNAP Benefits you should have received a letter regarding the changes to your individual SNAP Benefits. March 2023 the dollar allotment reverts back to pre-covid initial amounts. This means that if you were receivinag $250.00 before COVID and are now receiving an additional allotment of $200.00 you will lose that additional $200.00 effective March 2023. You will still receive the initial $250.00 if found to still qualify. The numbers used are only an example
Medicaid:
The COVID Relief act allowed for continuous coverage eligibility. This meant that even if you no longer qualified for Medicaid during the last three years, your Medicaid Benefits could not be terminated. Effective March 31, 2023 the unwinding started. This means that they will be removing the continuous coverage eligibility. If you are currently on Medicaid and no longer eligible your benefits will end sometime in the next 12 months. Once your benefits come up for annual renewal or if achange is reported then your benefits will be reviewed. If you are no longer eligible at that time, then your Medicaid Benefits will be closed. This will start with April Medicaid renewals.
Medicare:
This affects your Medicare coverage if you have been Dual Eligible. This means that you have been receiving Medicare and Medicaid. When you no longer qualify for Medicaid, you will lose your current Medicare coverage. You are then able to select new coverage that meets your needs. If you do not select the coverage, your current Insurance Carrier will select your coverage for you and put you on a plan that likely will not meet your needs. They will not be checking your providers and emdications to ensoure proper coverage. You will have a Special Election Period to select the coverage you wn that best fits your needs. The Putnam County Council on Aging, Inc. offers a wide array of services to support senior citizens age 60 and better. The Council on Aging has friendly and dedicated staff members who are committed to improving the quality of life for Putnam County senior citizens. When you come to the Council on Aging, we want you to fee at home at home, whether it's your first time here or if you have been receiving assistance for years.
Services include:
Aging in Place Home Assessments: Aging in Place simply means to be able to remain in your home as you grow older and your needs and abilities change. Staying in your home may mean making simple home modifications that can help you live more comfortably and safely. Our Certified Aging in Place Specialist can conduct a home assessment of common areas in the home such as entry ways, living room, kitchen, bedroom, bathroom, etc., and make recommendations on how to make your home more usable for current and future needs. A written report with recommendations and pictures is given to all home assessments clients. The Council on Aging Chore Program can also help make some of the recommended changes, if it falls within their skill level. The decision to make any changes in the home is completely up to the client.
Transportation for medical appointments and personal business: Our courteous and professional drivers provide transportation services for medical appointments and personal business in 7 agency-owned wheelchair accessible vans and 3 cars/SUV's. Medical transportation is priority and is available anywhere within Putnam County and to the adjoining counties of Allen, Defiance, Hancock, Henry, Paulding, and Van Wert. Transportation for personal business is available within Putnam County and includes trips to social service organizations, beauty salons, banks, pharmacies, grocery stores, and other essential errands. Hours of operations are Monday through Friday from 8:00 a.m. - 4:30 p.m.
Chore Assistance: Our skilled chore workers are available to offer help to older adults so they can rmain safe and secure in theri own home by providing assistance with seasonal needs, minor home
Outreach Services: Our professional and knowledgeable outreach staff are available to provide personalized help to link older adults to community resources in order to maintain their independence. The Outreach Specialist can help in navigating life changes, completing paperwork and "cutting the red tape: for Medicare, community referrals, fuel assistance programs, home delivered meals, filling out forms, interpreting hospital bills and so much more.
Loan closet for medical equipment: If a hospital stay, recent injury or just coming of age as a senior presented a need for small durable medical equipment, call our agency to see if we have what you need. Items available to loan include: canes, walkers, raised toilet seats, shower chairs, wheelchairs, stationary commodes and so much more. Items are available to loan for a few days, a few weeks or a few months. Call to check for availability.
Telephone Reassurance and Friendly Visitor Program: Maintaining social relationships can become more difficult as we age. To insure continued well-being of older adults who are isolated or homebound, our professional staff and volunteers provide regular telephone calls and/or a home visit to help older adults stay connected to their community and engaged in meaningful relationships.
Information and Referral: Our knowledgeable staff are available to connect older adults, their families and caregivers to community resources to help them meet their needs.
Matter of Balance: Falling is not a normal part of aging! Our trained staff and volunteers offer this 8-week, evidence-based program, which is designed to help older adults reduce their fear of falling and help increase activity levels by promoting exercise to enhance balance, strength and flexibility.
We Thrive Together: The Council on Aging partners with We Thrive Together to offer virtual programs to Putnam County older adults. This is a great way to talk, laugh, play, sweat, and connect from the comfort of home. Programming includes games, cards, bingo, exercise classes, virtual trips, trivia, and so much more. The Staying Connected service provides a FREE, daily check-in by phone for Ohioans age 60 or older. This service provides comfort and can connect you with aging network information and support. It's easy to get started and easy to use.
How the program works:
You provide the service with a working phone number and the time of day you wish to be contacted between 7:00 a.m. and 6:00 p.m. The service will then contact you during that time each day. When you received the call, you will be given the option to confirm that you are ok, an option to be transferred to learn more information about service options or given an option to connect for a friendly chat. If you do not respond, they will attempt three times to contact you. If you do not respond, they can then either contact a secondary number if you choose to provide one to them, attempt a live call to you and your alternate number or notify local law enforcement of an non-emergency service request.

Meals on Wheels program provides older adults with nutritious home delivered meals five days a week, Monday through Friday.
Meals are $6.75 and are nutritionally balanced and low in fat, sugar and sodium. Meals include the choice of 2% or skim milk or juice. Anyone is eligible to receive these meals. It could be individuals who are elderly, disabled, new mothers, or someone who doesn't want to prepare meals for themselves for any reason.
Government assisted programs are available and you may call for an assessment of need.
Hot meals are available Monday, Wednesday and Friday. Frozen meals are available for evenings, non-delivery days and weekends. Meals are delivered between 9:30 a.m. and 2:30 p.m. and clients must be home when meal is delivered. Meals cannot be left without the client being present.

Homestyle Direct's Home Meal Delivery program ships free, healthy, delicious meals directly to Medicaid recipients receiving the approved meal delivery waiver. This program serves seniors and people with disabilities in need in order to help them live independently at home.
A Medicaid recipient must first contact their Medicaid agency or Medicaid representative and request a meal waiver application. Once the completed application is returned to Medicaid, a subsequent evaluation will be scheduled to determine eligibility.

The DISH program is an innovative way for eligible older adults to enjoy a meal out once in a while.
Eligibility:
You must reside in one of the following counties: Allen, Auglaize, Hancock, Hardin, Mercer, Putnam and Van Wert
You must be at least 60 years old or a spouse under the age of 60 who resides in the home of an eligible participant who is also eligible for the program, as long as redeeming the coupons at the same time as the person who is 60years old or older. The person who is under 60 also needs to apply separately. Please indicate this on your application.
You must live in the community. if you reside in an Assisted Living or nursing facility, you will not be eligible.
You must not be on the PASSPORT program or receiving home delivered meals through the Area Agency on Aging 3 home delivered meals program.
All applications must be filled out completely. Applications not completed in their entirety will not be eligible for the program.
Please note, not all individuals that apply for this program will be eligible and not all will be enrolled. Enrollment for the DISH program is based off of many factors which include: age, nutritional risk, income status, living alone, minority status and other factors.

Driving Assessments
Service explanation: A guided in-home self-assessment conducted by a certified Beyond Driving with Dignity Professional serves as a tool for the older driver and his/her family in making appropriate driving related decisions. If the individual is a safe driver, we provide strategies on how to remain a safe driver as they progress through the aging process. If driving retirement is the appropriate decision, then we provide acceptable transportation alternatives, resources and a specific plan to ensure a smooth and successful transition from the driver seat to the passenger seat.
Eligibility: This service is for anyone that can benefit.

Service explanation: As we get older our reflexes and our ability to make quick decisions while operating a car naturally slow down. Changes in our abilities due to injury or illness can enhance these changes and result in poorer response time, peripheral vision and thinking skills. A driving evaluation is designed to help individuals and their families determine the ability to safely drive a motor vehicle.
Eligibility: Any individual who may be at risk for decreased visual, spatial and cognitive skills, as well as those wishing to validate their ability to operate a motor vehicle.
Make a referral: A physician prescription is required and can be written for A DRIVING EVALUATION. Appointments can be made by calling St. Rita's Outpatient Occupational Therapy Dept. at 419-226-9019.
Cost: With a physician prescription, MOST insurance companies will cover this service.
