What is it?
A legal document ensuring the site meets university standards. Required before starting clinical hours.
Who signs it?
Must be signed by someone at the site authorized to sign legal contracts (e.g., HR Director, Legal Team, etc.)
Timeline
Start early to avoid delays! The timeframe on completion is outside of our control. The time required to complete may vary depending on the type of site or facility (e.g., privately owned practice or large health care system).
How to find the Affilation Agreement form in CORE ELMS
When you log into CORE ELMS, navigate to Document Library on the left-hand side menu and scroll down to the Clinical Process Steps folder.
Under Requirements Step 5: Complete your site & preceptor's paperwork, you'll find information about how to download the Affiliation Agreement (see image below).
The Clinical Student Support Team will make every effort to facilitate a completed affiliation agreement as efficiently as possible. In some situations, it may not be possible to complete an affiliation agreement with a requested clinical site, in which case a clinical rotation cannot proceed with that site. Your Clinical Student Manager will provide you with support and updates regarding the affiliation agreement. Your patience and understanding of this important step in the process to enroll in your clinical courses is appreciated.
❗If the agreement cannot be finalized, you’ll need to choose a different site. Contact your Clinical Student Manager immediately.
