In Peel, there are two LHIN offices, one serving south Mississauga and Halton and one serving north Mississauga and Brampton/Caledon. It’s best for you to use the online locator and postal code to get the correct office (you can do this by googling LHIN).

Program Description

• provides information about local community support service agencies and can link people to the agencies to arrange services
• case managers or care coordinators case provide advise about what services your LHIN can provide and what’s available in your community. Government-funded services are delivered by health professionals and personal support workers who are under contract with your LHIN.
• coordinates services for seniors, people with disabilities and people who need health care services to help them live independently in the community
• conducts assessments for assistive devices for the bath or mobility

Labatt Better Together Quality of Life Grants

Program Description:

• This program provides funding for support services to relieve conditions associated with aging, such as personal care, housekeeping, meals, nursing and transportation.
• Grant amounts typically range from $200 - $4000.

• Adult Canadian resident with low income

Adult Protective Service Worker (APSW) Program

Program Description

This program supports adults with a developmental disability who are living on their own. The goal of the program is to help them live as independently, safely and securely as possible in the community. Adult Protective Service Workers work directly with the person who has a developmental disability to help them find and maintain supports, including:

• medical and dental care
• counselling for emotional support during stressful life changes
• social and housing

Eligibility Requirements

Adults aged 18 years or older who have a developmental disability are eligible for the Adult Protective Service Worker program if:

• they live in Ontario
• they live on their own in a community setting or plan to move to a more independent community setting
• they have limited or no significant social supports
• need, and request, the type of assistance that APSWs provide

This program access is managed through developmental services Ontario

Note: there is no upper age limit for eligibility for the Adult Protective Service Worker program.

  • Caregiver Support and Respite

  • Case Management for frail Seniors over 55 years of age
  • Behavioural Services for Seniors who have cognitive impairments due to dementia, addictions, or neurological conditions

Eligibility Requirments:

• Anyone who speaks English or one of the South Asian Languages
• Seniors 55+
