Open Records Request

To assure adherence to KRS 61.870 to 61.884 regarding public records, the following policy is applicable to the dissemination of public records.

These processes are to be followed by all NKY Health staff that receives a public records request (in writing or verbally.)

1. The principal office of the Northern Kentucky Independent District Health Department is 8001 Veterans Memorial Drive, Florence KY 41042.

2. The Official Custodian of Records is the District Director of Health (or her/his designee).

3. The REQUEST TO INSPECT PUBLIC RECORDS form must be filled out completely and be returned to the principal office.  A letter containing the same essential information as the form can, in some cases, be accepted in lieu of the actual form.

4. The documents requested must be very specific and require approval from NKY Health's Official Custodian of Records before being released.

5. The completed form must be forwarded to the Official Custodian of Records.

6. When the documents are provided to the requestor, a charge of 10 cents per page is to be collected and forwarded to Accounts Payable, unless waived by the District Director of Health in cases where the total payment would be less than $5.00.  There is no charge for electronic copies of public records, unless a media such as a compact disc is required.

7. Medical records requests should be handled by Clinical Services and follows a slightly different process than what is described in this policy.  The first copy of a Medical Record requested by a patient is free of charge.  All additional copies of the Medical Record are to be charged at the 10 cents per page rate.
