The Chemical Pathology Melaka Cluster Pathology Service provides diagnostic and consultative services for patient management in Hospital Melaka, Hospital Alor Gajah, Hospital Jasin, other government hospitals and health clinics in Melaka. It operates 24 hours daily including weekends and public holidays.
Head of Unit
The diagnostic services are divided into:
Urgent Test (STAT).
Routine Tests – all chemistry tests that are offered during and after office hours.
Specialized Tests in Hospital Melaka – Immunochemistry testing, Therapeutic Drug Monitoring (TDM) testing and Cord Blood TSH Screening.
Screening and Confirmation testing for Drug of Abuse (Opiates and Cannabinoids). DOA screening testing are offered at all hospitals in Melaka Cluster Pathology, however confirmation testing are only in Hospital Melaka.
24 hours daily including weekends and public holidays.
On-call Medical Officer and Pathologist are available for clinical consultation or assistance and Scientific Officers are available for technical consultation after office hours, contactable via hand phone or by hospital telephone operator.
All requests must be made using the PER-PAT 301 Form except for Drug of Abuse (DOA) request for Medical check-up and Medico legal cases, Cord Blood TSH Screening test and TDM testing.
National Congenital Hypothyroidism Screening Form – to be used for Cord Blood TSH Screening.
Therapeutic Drug Monitoring (TDM) Request Form – to be used for TDM testing.
Borang Permintaan Ujian Pengesanan Dadah Dalam Air Kencing (UPD-1) dan Borang 'Chain-Of-Custody' (COC) - to be used for Drug of Abuse (DOA) request for Medical check-up and Medico legal cases.
Additional verbal requests using existing primary specimens shall be followed by new request form with details of requester, date and time of request. Acceptance of this requests are subjected to specimen stability or according to unit criteria.
Blood for clinical chemistry and immunochemistry assays shall be collected in plain tube. The anticoagulant used for blood glucose and lactate are Sodium Fluoride / Disodium EDTA or Potassium Oxalate + Sodium Fluoride while anticoagulant used for blood HbA1c, Ammonia and Troponin i (in Hospital Jasin and Hospital Alor Gajah) are K2 EDTA.
A minimal tourniquet pressure should be applied in performing venesection to avoid forcing free fluid out of the distal capillaries resulting in haemoconcentration and spurious elevation of protein and protein-bound substances e.g Calcium and free-T4.
Many patients requiring clinical chemistry investigations are receiving intravenous infusion. Venesection done proximal to the infusion site will cause contamination. This will result in a wrong representation of the actual concentration of the analytes.
Do not mix blood sample from different containers. Certain containers e.g., KF+Na2 EDTA or K2 EDTA tubes contain Potassium salts and chelating agents which will cause error in the result for Potassium and Calcium analysis.
