. Final stage about Statins
• Cholesterol is a fatty material in your blood which is mainly made in the liver.
• It plays an important role in how our cells work and helps your body make vitaminD and hormones which keep your bones, teeth and muscles healthy.
• Having too much cholesterol can clog your arteries, causing problems like heart disease,so we use Statins to treat it.
- Statins atins are a class of medications primarily used to lower cholesterol levels in the blood.
- Medications of Statins used to lower or reduced blood cholesterol levels (Atorvastatin, simvastatin and Rosuvastatin).
Some images about Statins and groups of Statins
- Statins atins may be recommended if you have cardiovascular disease (CVD) or at a high risk of getting it in the next to years.
- Statins can be also used to treat familial hypercholesterolemia.
● Lipitor( Atorvastatin).
● Crestor ( Rosuvastatin).
● Zocor ( Simvastatin).
● Pravachol ( Pravastatin).
● Lescol ( Fluvastatin).
- Mechanism of action of Statins: mode of action is primarily via inhibition of HMG-CoA reductase ( hydroxy methyl glutaryl- coenzymeAreductase).
- By blocking this enzyme, Statins reduce the amount of cholesterol synthesized by the liver, leading to the lower levels of LDL "bad" cholesterol in the blood stream.
- Muscle pain with a "nocebo" effect contributing to its perception.
- Hepatoxicity/ Headache.
- Myalgia / Myopathy.
- GI effects (diarrhea, flatulence).
- CPK increase.
- Avoid in pregnancy.
● Vitamin B .
● Garlic .
● Melatonin .
● CoQ10 .
- The pharmacodynamics of Statins primarily involve inhibiting the enzyme HMG-COA reductase, which is crucial for cholesterol synthesis in the liver.
- By blocking this enzyme, Statins reduce the production of cholesterol, leading to decreasedecreased levels of LDL cholesterol in the blood stream.
- Additionally, Statins may have anti-inflammatory and plaqueplaque-stabilizplaqueplaque-stabilizibpng effects, contributing to their cardiovascular benefits beyond just lowering cholesterol levels.
- Statins prevent heart disease by lowering levels of LDL (low-density lipoprotein) cholesterol, often referred to as "bad" cholesterol.
- By reducing LDL cholesterol, Statins help prevent the buildup of plaque in the arteries (atherosclerosis) ,which can lead to narrowed or blocked arteries and increase the risk of heart attacks and strokestrokes.
- Aditionally, Statins may have other beneficial effects on cardiovascular health, including:
1- Reducing inflammation : ( anti-inflammatory).
2- Improving Endothelial Function: ( lininglining thethe blood vessels, is an early step in the development of atherosclerosis).
3- Stabilizing Plaque: ( in thethe arteries, makingmaking them less likely to rupture and cause a heart attack and stroke).
4- Antioxidant Effects:( can hhelp reduce oxidative stress and damage to blood vessels).
- For individuals who cannot tolerate Statins or prefer alternative options, there are several alternatives to consider :
1- Diet and Exercise: (can help improve cholesterol levels and overall cardiovascular health).
2- Other Medications: ( there are other classes of medications, such as Ezetimibe, Bile acid sequestrants, Fibrates, and PCSK9 inhibitors, thatthat can be used either alone or in combination with Statins to lower cholesterol levels).
3- Red Yeast Rice: (this is a traditional Chinese supplement that contains naturally occurring Statins).
4- Omega-3 Fatty Acids: ( such as fish oil that have effects on lowering triglyceride levels and offer some cardiovascular benefits).
5- Plant Sterols and Stanols: ( these compounds, found in certain foods).
6- Lifestyle Changes: ( as Quitting Smoking, Limiting alcohol intake, and managing Stress).
- Statins are one of the most controversial topics in healthcare and the noise around the topic has been the cause for so much confusion and suffering.
- A few case reports were published in the early 2000s that Statin intake led to memory loss and other cognitive issues, which in turn led to FDA issuing a black box warning on Statins in 2012 for possible adverse effects of Statins on cognitive performance.
- Now, these concerns are the reasons why you still hear that Statins are bad for the brain but we have bad tremendous amount of data since then, and just recently a very through systematic review and meta analysis on the use of Statin and risk of dementia and Alzheimer's showed that Statin use not only did not increase the risk of dementia, but it reduced it by 20% and it reduced the risk of Alzheimer's disease by 32% and it make sense because despite the noise high cholesterol is one of leading causes of strokes, vascular dementia, and even Alzheimer's definitely for cardiovascular diseases.

• It's important to take your medication as prescribed and your doctor may recommend you take your Statin at a certain time every day as this keepkeeps your blood levels even.
• If you forget to take your medication, take your regular dose the next day.
• Don't take extra of your medication and if you take too many Statins, contact a pharmacist or call NHS 111.
• Top tip - many patients take Statins before they brush their teeth at night as part of their routine so they don't forget ( it'sit's also when most of your body's cholesterol is produced).
