CTE 37 Initial Form - This form is used at the start of your teaching and coursework at KSU.
CTE 37 Renewal Form - This form is used if you cannot complete the program in 2 years and need additional time. Please note: At least 1/2 of the required courses must be completed prior to submitting the renewal form.
CTE 37 Advanced Form - This form is completed at the end of your KSU coursework.

Graduate Plan of Study for Earning a Master's Degree - This is the form you must submit to the graduate office near the start of your coursework in order to proceed through the master's program.
