This waiver is required for any live/interactive platform. A student must have one on file for each platform per academic year * 

This waiver may be asked to accompany your order request if the content is deemed above age level

This waiver will be required for certain vendors that do not fully meet all of the California state background requirements 

This waiver is required for all items requested through the tech catalog as well as Kindles. 

This form may be asked for if your student is requesting something that needs additional supervision, such as but not limited to, dissection kits. 

This waiver can be completed for any curriculum platform
