SchoolsPlus is an integrated service delivery model for services offered to children, youth and their families. The vision of SchoolsPlus is to serve as a center of service delivery, bringing together professionals, community partners, and other service providers to help children, youth and their families overcome barriers, and navigate services while seeking opportunities for enrichment to help students fullfill their potential.
SchoolsPlus is part of Nova Scotia's child and youth strategy, Our Kids Are Worth It.
SchoolsPlus assists with accessing services outside of the scope of practice of the school counsellor
They act as liaisons between the school and community and advocate, coordinate, and expand services for students and families. They help families navigate the system and get the services they need.
SchoolsPlus is involved with families outside of the school setting. This work can occur in the community, home, or by attending medical appointments at facilities within the Tri-Counties, IWK, etc.

A referral for SchoolsPlus comes primarily from the principal and school Teaching Support Team.
Service providers, students, and families may also contact the SchoolsPlus Facilitator for their school to make a referral.
The SchoolsPlus Facilitator is then invited to school Teaching Support Team meeting. The degree of the response will depend upon the level of need identified at the time of referral. Sometimes this will mean a simple sharing of contact information to connect families with the services they need.
At other times, the Facilitator may organize a meeting with a number of service providers to create an action plan with a family. We call this action plan a Comprehensive Service Plan.
Students may need a Comprehensive Service Plan when:
they need to be connected to multiple services or further services are needed for success
to ensure accountability that supports get provided
there are barriers that prevent access to services, including systemic barriers
all available services and solutions have been exhausted, but creative and cooperative solutions must be developed
Coordination of supports and navigation of systems and services in school and community
Communication between students/families, schools, supports and services
Making appropriate referrals to other agencies
Empowering, supporting, and encouraging families and students to be their own advocates
Partnering/collaborating with professionals and organizations to deliver programming
Accessing funds to support students with medical/dental needs, recreactional opportunities, transportation, and programming
Providing a different lens with a unique, client-centered and holistic framework
Being available to students/families thorughout the year and strive to fill gaps that occur beyond the school day
