Add Form 1099-G Listing Page
Once you are within your Tax Return, look for Forms In Return on the left of your screen. Scroll down to the bottom.
Click the "Federal" drop-down selection.
Click the "Income" drop-down selection.
Scroll down in the list of "Income" Forms and click "Form 1099-G Input Page".

This is the preprinted year displayed under the "OMB" number.
DO NOT enter the year from box 3 here.
Payer's Name:
Enter the Payer's Name here.
Payer's Address:
Enter the Payer's Address here.
To enter a Foreign Payer Address, click the sentence "To enter a foreign address, click here".
Payer's Phone Number:
Generally, this is an optional field.
Payer's Employer Identification Number (EIN):
Enter the Payer's Federal Tax Identification Number.
This is a required field. The number is 9 digits and can be called a TIN.
Recipient's Social Security Number:
Enter as shown on Form 1099-G.
Recipient's Name:
Enter as shown on Form 1099-G.
When both taxpayer and spouse names are present, enter name that matches the SSN listed.
Recipient's Address:
Enter as shown on Form 1099-G.
To enter a Foreign Address, click the sentence "To enter a foreign address, click here".
Account Number:
This field is optional.
Unemployment Compensation & Repayment:

If you repaid unemployment compensation received in the current tax year, enter it here.
This amount will be subtracted from the total reported on Form 1040.
