Orientation Forms

Click HERE to be directed to the FHE webpage that includes a variety of forms and information pertaining to starting school. This includes the student Parking Permit Form (for students who drive), suggested supplies for school, yearbook ordering information, and links to get involved in any of our several Boosters groups. 


Lunch Procedures

Hot lunches are provided at a reasonable cost for students to purchase daily. Student ID cards will be used as debit cards for lunches. Parents are to send in checks made out to Forest Hills Schools and this amount will be credited to the child’s account, or parents may create an account and pay online HERE .

Free and reduced lunches are available to qualified families. Information may be obtained in the main office. Completed forms are to be returned to the main office. Food Service will notify families if you qualify. You can also contact the Director of Food Service directly at (616) 493-8774. All information is confidential.


ID Cards

New students enrolling in the fall will receive an ID card when their school picture is taken at orientation. New enrollees beginning after the start of the school year may obtain their ID from our Security Officer. Students are issued picture ID cards that are used as their lunch debit card, library cards and allow access to the Internet. Parents will pre-pay into their child’s lunch account and the child will be able to charge against that balance. Students who lose their cards should report to the main office for a replacement. A $5.00 fee will be charged to replace the card.


Cell Phones/Electronic Devices

Cell Phones/Electronic devices are not permitted during the school day and will be confiscated. ** Refusal to give-up device to staff member may result in more severe consequences. First offense, the student may pick up his/her device from an administrator at the end of the day. Second offense will result in the confiscation of the property until claimed by a parent/guardian.Third offense will result in the confiscation of the property, phone contract (check-in/checkout of device for remainder of semester) and thirty days of social probation, which prevents a student from attending school dances, athletic events, and other school functions at home or away. Violation of phone contract may result in suspension from school.
