REGISTRATION PROCESS FOR STUDENTS

If you would like paper copies of any of these documents, please visit:

College and Career Center (Room 1201)

Forms to be filled out:

• Student Permission Form (Mandatory--this is a must!)
• Student Matching Form (Optional)
• Student Tracker Form (Optional)

In order to become an ASPIRE Student, you need to have your parent or guardian fill out the Student Permission Form and return this form to the ASPIRE Coordinator (Rebecca Kelsay). The other two documents (Student Matching Form and Student Tracker) are optional. The Student Matching Form will help the ASPIRE Coordinator to best match you with a Volunteer Mentor so please fill this out to better assist you! The Student Tracker form will need to be filled out but can be done during your first meeting with your Volunteer Mentor.
