Test Examiner's Security Agreement

All school staff performing the role of test examiner or assisting in the administration of a state or national standardized test must have a signed copy of this form on file with the school's testing coordinator. The school testing coordinator must inform the principal and the district testing coordinator should less than 100% of the required security agreements be submitted by the end of September or prior to any scheduled state or national standardized test.

This is a slideshow that testing coordinators and teachers must view each year to review the importance of test security. 


Testing Irregularity/Security Violation Reporting Form

Use this form to report any testing irregularity or security violation. Submit the form on the day of the event. You should call the testing office prior to submitting the form.


Student Testing Code of Conduct

It is important that each student’s performance on district and state tests reflect his or her  own achievement, ability, and work. Therefore, it is expected that all students follow this Student Testing Code of Conduct. Prior to testing, teachers should review the statements  below with students, making sure they understand each. 


Access List 

List the names and titles of each person with access to the secure test storage area, whether or not they have a key to the room, and what level of access they have to the test materials. State tests must remain shrink-wrapped until the first scheduled day of each test. No test may leave the secure storage area until the first scheduled day of each test. Also, please make sure that all school staff that may be the initial delivery contact for testing materials (e.g. school principal, plant operator, school office staff, lunchroom manager) know to contact you, the testing coordinator, upon delivery in order to see that tests are secured immediately.


Accountability Form

The Accountability Form is used to ensure the secure transfer of test materials. Every item must be accounted for and returned at the end of the testing window. This form should include the people receiving and transferring the test materials, the count of items, and the actual serial numbers of the materials being transferred.
