Record Corrections
When submitting a request for a patient record to be corrected (images/study corrected or moved, wrong patient, different patient records combined on same MRN), please submit a ticket to Service Desk via email by listing the following:
EMAIL TO: Servicedesk@simonmed.com and risteam@simonmed.com, CC: Compliance@simonmed.com
With the SUBJECT LINE: “ATTN RIS/Data Integrity – Patient Record Correction”
Please include the following information in the BODY OF EMAIL:
Ø MRN(s) impacted
Ø Full Name (for each patient impacted, if more than 1)
Ø Patient date of birth
Ø Accession(s) #
Ø Date of Service
Ø Details – What happened, why did it happen (if known)
Ø Other comments/information
This will route your request automatically to the correct team to begin getting the correction started.
