WHITE COAT CEREMONY
☐ Informed my family about date and time for the Ceremony.

CRIMINAL BACKGROUND CHECK
☐ Completed the CBC through Certiphi Inc.
- Deadline is June 25th.
FINANCIAL RESPONSIBILITY AGREEMENT
☐ Completed this agreement through SIS.
- Deadline is June 25th.
TRANSCRIPTS
☐ Contacted my college(s) to have official transcripts sent to Admissions.
- Deadline is June 25th.
IMMUNIZATION RECORDS
☐ Completed the required forms and updated immunization records on MyHealthConnect. Deadline is June 25th (but don't wait until the deadline!).
STUDENT MEDICAL PLAN
☐ Reviewed the Student Medical Plan option.
COMPUTING & LAPTOP INFO
☐ Reviewed the Laptop BYOD policy.
☐ Reviewed and completed the computer set-up instructions.
☐ Registered for a computer orientation session.
☐ Reviewed the information about the class roster picture that can be taken after computer orientation. Information found in the Laptop Info.
FINANCIAL AID
☐ Formally accepted my financial aid award by June 18th.
☐ Followed the procedures for signing the Master Promissory Note and completed entrance counseling through MyFinancial portal by June 18th.

STUDENT ID
☐ Reviewed the instructions for obtaining my student ID.
☐ Optional (but highly recommended) – submitted my picture to Access Services for my Student ID.
PARKING PERMITS
