Google Forms let you: Collect and organize information big & small they can be used as surveys, invitations, tests and lots of other things.
Objective: Create a Google form that will survey others in the class then graph the results using sheets.

Open Google Chrome then Log in to your Google account Click Apps then Drive
Click New > More then Google Forms

Google forms has several types of questions we are going to create a form with three questions of three different types. The first question will be a favorite question.
You need to pick a question that has a maximum of 10 different responses. If you can think of your own question great.
Replace "Untitled form" with Student Survey
Type your question
Help text can be used if your question is unclear and needs some explanation.
Select Choose from list as Question Type
Add up to 10 Options for responses to your question
You can change the order or remove the options
Here are some examples of favorites:
What is your favorite animal?
What is your favorite city in the United States?
What is your favorite color?
What is your favorite day of the week?
What is your favorite drink?
What is your favorite food?
What is your favorite holiday?
What is your favorite kind of book?
What is your favorite kind of movie?
What is your favorite kind of music?
What is your favorite kind of pizza?
What is your favorite season?
What is your favorite sport?
What is your favorite television program?
What is your favorite time of day?
What is your favorite TV program?
What is your favorite vegetable?
What was your favorite subject in school?
Select Required question and click Done
Now we will add our second question. This question will have a number as a response.
Try to think of a question that will have most answers under the number 10.

Select Scale from list as Question Type
In Scale select the range of responses
In the label option type in "or More" if the responses may be over your highest scale.
Select Required question and click Done
Now we will add our third question. This question will have a Grid rating.
Try to think of a question that will have have several related things that can be rated
If you can think of your own question great.
Type in your question and type Help Text so they know your grid rating system usually 1 being lowest and 10 being highest.
Select Grid as your question Type
Type in Row Labels for the items you wish to be rated
Keep the number of labels under 10 total
Add Column Labels 1 through 10
Select Required question and click Done
Change Theme of Form to make your form looking nicer click the theme button and choose the theme you like

At anytime you can View live form to look at what your form looks like
Choose Publish and allow responders to edit and click Send form
You have several options on how to send the form.
You can create a link to share
The easiest way is to send all the people you want to take your survey an email link.
Type in all the names of the students in your class and send the email to them
If your teacher has a google class room then send your teacher a email and they can post all the surveys in the class room.
