A. A student shall submit a request to the Department Chair/Program Director (cc: Registrar, registrar@ateneo.edu)
The email should include the following:
1. Subject Title: REQUEST FOR LOA - Student’s Last Name - Degree Code (e.g. REQUEST FOR LOA - Dela Cruz - MA POS)
2. Student’s Name
3. Student’s ID Number
4. Degree Program
5. Current SY and Semester
6. Duration of Requested LOA
7. Reason(s) for LOA
8. Type of scholarship, if any. (e.g. DOST ERDT)
NOTE: If the reason for the LOA is medical in nature, the student should follow the guidelines for securing and submitting a medical certificate located at the end of this document.
B. The Department Chair/Program Director replies to the student to signify approval/disapproval of the request (cc: Registrar, registrar@ateneo.edu).
C. If the request is disapproved, the Department Chair/Program Director shall inform the student of the decision and state the reason/s for the disapproval.
D. The Registrar shall update the Student’s status to “LOA”.
Guidelines for Submission of Medical Certificate for LOA
The concerned student shall reach out to his/her physician via email (preferably, using the latter’s official email account) and request for the issuance of a medical certificate addressed to the Department Chair/Program Director. In so doing, the student must make sure he/she copy-furnishes the Department Chair/Program Director.
The email should state:
“I am expressly authorizing the University, through the Department Chair/Program Director, who is copied on this email, to contact you for any questions or clarifications regarding the medical certificate you may issue.”
Important Note: The student is responsible for furnishing a copy of the medical certificate to other concerned offices or agencies, if needed (ex: OAVPGE, employer, scholarship provider, etc.).
The physician shall send a scanned or electronic copy of the medical certificate to the student, while copy-furnishing (cc:) the Department Chair/Program Director.
