APU Accessibility & Disability Resources
Accessibility and Disability Resources coordinates a number of direct services for undergraduate and graduate/professional students with disabilities or accessibility challenges. Accommodations are individualized based on a documented disability and the functional limitations of each student.
APU CARES
Student in Crisis Notification Form
Students in academic or emotional distress are often first identified by faculty, staff, family, or friends. Although many students will choose to seek and access campus support services on their own, others may not be aware of what APU has available to help or know how to get connected.
If you have recognized a student who is experiencing a significant academic issue and/or emotional crisis, please click on the button below, and complete and submit the corresponding electronic form. If you are referring multiple students, please complete one form per student.

Campus Safety Silent Informant Form
If you have any information about non-emergency criminal activity involving an APU student, faculty, or staff member and would like to keep your identity confidential, please utilize the Campus Safety Silent Informant Form.
APU Center for Diversity, Equity & Inclusive Excellence
A bias incident is any intentional or unintentional act or behavior directed toward an individual or group based on any facet of the individual’s or group’s identities. Azusa Pacific is committed to fostering a welcoming and safe environment for all members of the university community and encourages all members of the university community who have witnessed or experienced a biased incident to report it.
APU Title IX Office
If you experience any kind of behavior that may violate Title IX, which prohibits sex discrimination, including sexual harassment, sexual assault, and sexual violence, during educational programs and activities. APU encourages those who have experienced any form of sex discrimination to report the incident promptly, to seek all available assistance, and to pursue University disciplinary proceedings and, where applicable, criminal prosecution.
APU Student Health Center
The Student Health Center supports the university by evaluating, treating, maintaining, and promoting optimal physical, mental, and spiritual well-being of APU students. The Student Health Center serves as an advocate for patient care and promotes individual empowerment to enhance healthy lifestyle choices among patients.
APU University Counseling Center
Azusa Pacific University Counseling Center (UCC) exists to empower students to realize their personal and academic potential by promoting psychological, social, and spiritual wellness through Christian counseling and outreach services.
