Application for Employment Certificate
Minors wishing to obtain part-time employment must have their Employment Certificate, also known as "working papers." Minors may apply for working papers in the district they go to school, the district where they live, or the district where they are to be employed. Working papers are available for 14-15 year olds (blue card) and 16-17 year olds (green card).

WORKING PAPER REQUIREMENTS & DIRECTIONS
1. Download the 'Application for Employment Certificate' form attached above and save it to your computer. Parents/guardians and minors must complete Parts I, II and III. Once completed, print and sign the application.
2. The student should bring the completed form (including parent signature) to the Counseling Center. The student must be present.
4. Once the application is reviewed and verified* by a certifying official, the working paper certificate will then be completed.
5. The student must sign the working paper certificate in the presence of the certifying official.
*The student must have documentation on file that a physical examination was completed within the last 12 months (see Part III below).
APPLICATION SECTIONS
Parts I, II and III of the application must be completed.
PART I - Parental Consent - Parent or guardian signature must be included.
PART II - Evidence of Age - Westhill High School students may use their schooling record as proof of age
PART III - Certificate of Physical Fitness - Students must have had a physical examination within the last 12 months either by the school or a private physician. If you had your exam completed by the school physician within the last 12 months, this will be on file with the school and no further action needs to be taken. However, if you did not have an exam by the school physician or did not provide the school with the results of your physical done by your private physician, then you must contact your physician for this documentation and provide it to the school's certifying official.
TYPES OF WORKING PAPERS
There are three types of employment certificates, all of which are applied for on Form AT-17 (Application for Employment Certificate).
Non-factory Employment Certificate - This certificate is issued to all qualified applicant who are attending school and are from 14 to 16 years of age. It is valid for part-time work when school is in session, and for full-time work during vacation periods.
Student General Employment Certificate - This certificate is issued to all qualified applicants who are attending school and are from 16 to 18 years of age. it is valid for part-time work when school is in session, and for full-time working during vacation periods. It is valid for factory work.
Full-Time Employment Certificate - This certificate is issued to a minor who is leaving school, or has graduated from high school. Applicants must be from 16 to 18 years of age. The certificate may be issued to a minor under age 16, if the applicant is a high school graduate. In that case, however, it is not valid for factory employment.
