Forms for Juniors and Seniors

Below you will find information on post high school planning forms for juniors and seniors. Students can find these forms in the Junior and Senior Google Classrooms.

Transcript Request Google Form (TRF)

Once you complete this form, you must let your counselor know and ask to schedule a meeting with them to review at least 5 school days before your deadline (no action is taken unless you meet with them).

- The TRF form needs to be completed for only ONCE for colleges you are applying to using the Common App (regardless of how many colleges you are applying to on the CA).

- The TRF form must be completed for ALL other colleges NOT on the Common App.

Counselor Letter of Recommendation Request Google Form

If a counselor letter is required, the Senior Recommendation Questionnaire and Parent Evaluation Google Forms need to be completed. Once these are submitted, you will need to complete the Counselor Letter of Recommendation Request Google form and schedule a brief meeting with your counselor to review at least 14 school days before your deadline (no action is taken until you meet with them).

- The CRF form needs to be completed for only ONCE for colleges you are applying to using the Common App (regardless of how many colleges you are applying to on the CA).

- The CRF form must be completed for ALL other colleges NOT on the Common App that require a Counselor LOR.

Faculty Evaluation Form (blue form)

Juniors ask three (3) junior year teachers in the Spring if they would feel comfortable completing a Faculty Evaluation form on their behalf. This confidential form provides counselors with valuable feedback that assists them in writing letters of recommendation. These forms are not shared with colleges.

Parent Evaluation Google Form (yellow form)

Seniors ask their parent or guardian to complete this form on their behalf. This form provides counselors with valuable feedback that assists them in writing letters of recommendation.

Senior Recommendation Questionnaire Google Form (aka Brag Sheet - green form)

Seniors complete this comprehensive form to share with their counselor information about their high school experiences.

Scholarship Application Processing Google Form

Complete a Scholarship Application Processing (SAP) Google Form for all scholarships you are applying to (if a transcript or counselor letter of recommendation is required). Once you complete this form, you must let your counselor know and ask to schedule a brief meeting with them (no action is taken unless you meet with them).
