Annual Paperwork

Every fall* the annual paperwork must be completed for your child's records. 

Prior to fall 2023, the forms were the brightly colored pages (pink, lime and purple). The pink annual update will be distributed as a paper form but the other two forms are now electronic.

*If your family transitioned to Elysian after the start of the year, we ask that you complete the 2 forms within the first week of enrollment.
