fever with a temperature of 100.0 degrees or more
sore throat and fever
uncontrolled cough
difficulty breathing
wheezing
undiagnosed rash and/or sores
vomited or had diarrhea in the past 24 hours
eyes that are red and sore, with yellow or green discharge
stiff neck, with or without headache, accompanied by a fever
Signs and/or symptoms show improvement
Fever-free for 24 hours without use of fever-reducing medications
24 hours after starting antibiotics
Zero vomiting and diarrhea for 24 hours
If your child has a communicable disease (Chickenpox, Covid-19, Influenza, Pink Eye, etc.)
If your child missed school to attend a doctor's appointment
At the request of school office staff or nurse
If your child has a reoccurring appointment, one note for the year is sufficient. The wording of the note from the provider should state the student attends a regular appointment weekly/biweekly/monthly from start date to end date, ex. "Jane Doe has a regular appointment every week from 9/1/24 to 5/24/25 with provider's name." Specific dates or days are not necessary.
Medications:
Over the counter medications (Tylenol, Ibuprofen, etc.), need to be given through the school nurse. Download the OTC form and bring it to the school with the medication.
Prescription medications need to be given through the nurse. Download the prescription medication consent form and it needs to be signed by your child's healthcare provider before the medication can be brought and given at the school.
For asthma and inhalers, download the "Asthma Action form" and it needs to be filled out by your child's healthcare provider.
All allergies requiring an epi-pen need to have the "Anaphlyaxis Action Plan" filled out and signed by the healthcare provider.
