Advocacy & Benefits Counseling for Health, Inc. (ABC for Health), is a Wisconsin-based, nonprofit public interest law firm that promotes health equity and social justice. ABC for Health is dedicated to ensuring health care access for children and families. ABC for Health’s mission is to provide health care consumers with the services and support they need to navigate a complex health care financing system. 

If you have questions about aging or living with a disability, your local aging and disability resource center (ADRC) can help. The ADRC provides information on a variety of programs and services. That includes long-term care options. The ADRC also helps people apply for programs and benefits, including publicly funded long-term care. 

Founded in 1978, Agrace is a nonprofit, community-supported health care organization providing personal care, adult daytime care, supportive care, hospice care and grief support to people who are aging, seriously ill, dying or grieving. Every day in southern Wisconsin, more than 1,700 patients and clients trust us to see them through very difficult times. 

The ARC Center for Women & Children is a community-based, women's-specific, day treatment program for adult women with substance use disorders who may or may not have children. Services are provided for up to five days per week.

We operate a grocery-style food pantry that serves food-insecure households throughout Dane County. We offer a free Community Meal two Saturdays each month and career development opportunities in the Kasieta Center. The commercial-grade Prairie Kitchen offers nutrition education and is home to our Kitchen to Table initiative. 

Birth to 3

The first three years are the most important building blocks of a child’s future. The Wisconsin Birth to 3 Program is an early intervention special education program. It helps children under the age of 3 who have delays or disabilities. “Early intervention” is the term used for the services and supports for babies and young children and their families. 

My Choice Wisconsin is a managed care organization with a passion for putting our members first. With five programs across Wisconsin, members receive long-term care and health services to help them live their best lives independently in their own communities. My Choice Wisconsin works collaboratively with local agencies and healthcare providers to ensure our members have ample choices and high-quality support and services from a diverse network.

As we assist patients seeking safe, necessary health care options, we actively share patients’ stories with policymakers and people who design services to ensure that systems become more responsive to the lived experiences of people most affected by health, social, and economic problems.  

We provide our customers with a full range of affordable options, including prescriptions (Eastside only), herbs, supplements, sexual health, natural body care products, and homeopathic remedies. Many of our products are sourced from small, local businesses and companies that prioritize sustainability over maximizing profits. We take time to learn about the products we sell, and we enjoy educating our customers so that they can find the information and products they need. Community Pharmacy was founded on the principle that information and choice are crucial in making sound health decisions. That same ethos guides all of our actions today.

ConnectRx Wisconsin is a program that provides support to certain pregnant patients with social needs in Dane County. Patients who qualify will get support from a community health worker to address their needs. 

CCS is meant to help with recovery. The program works to stabilize and address mental health and substance use concerns, which include self-managing physical health and social health and meeting basic needs, such as housing, education and work. Those enrolled in CCS take control of their treatment and recovery. They work with a team to decide which services and supports will help them reach their goals. These services and supports may include:

Domestic Abuse Intervention Services (or DAIS) believes that a world free from violence is possible. We work to make Dane County, WI, a safer, more just, more equitable place for people who have experienced domestic violence, their children, and the people who love them. DAIS, helping Dane County since 1977, strives to empower those affected by domestic violence by advocating for social change through support, education, and outreach. To make this happen, we offer a wide array of crisis intervention and community education/prevention programs, including a 24-hour Help Line, programming for children from violent homes, legal advocacy, support groups, and emergency safety planning. DAIS is also the only domestic violence shelter in all of Dane County.

Foundations is a State Certified Outpatient Mental Health Clinic Specializing in In-Home Therapy For Children, Adults and Families. Foundations Counseling Center Inc was started in 2004 by Cristie Harbour, MS and Alisa-Kelly-Martina, MSSW, LCSW. Foundations Counseling Center Inc is a private outpatient mental health clinic operating out of Belleville, WI. Foundations Counseling Center Inc provides a wide range of traditional outpatient mental health services. In addition to these more traditional services, the clinic also provides Intensive In-Home Family Counseling for children and families, and in-home therapy for adults.

The Department of Human Services is a governmental agency authorized by Wisconsin State Statutes. The Department evaluates eligibility and administers a variety of Social and Human Services programs to persons living in Green County. 

Services Provided:

Head Start is a comprehensive child development program that focuses on promoting school readiness through positive social and educational skill development. Head Start services children ages 3-5 living at or below the Federal poverty guidelines. Head Start provides health and nutrition services, mental health support, family support services, and many opportunities for parent involvement ad leadership. There are additional support services for children with disabilities. Reach Dane is a community partner of 4K in DeForest, Madison, Monroe, Sun Prairie and Verona school districts. 

The River is South Central Wisconsin’s busiest food pantry. Services include free groceries and freshly prepared meals for pickup or delivery, online grocery orders, mobile meals, and emergency food lockers. The River also redistributes rescued food and resources to local partners to increase access while minimizing waste. Since 2006, The River has grown to serve over 2,500 people every week in pursuit of its vision: a fully nourished community. 

At Planned Parenthood our mission is to ensure all people have access to the care and resources they need to make informed decisions about their bodies, their lives, and their futures. Founded in 1916, Planned Parenthood is a trusted health care provider, educator, and passionate advocate here in the U.S. as well as a strong partner to health and rights organizations around the world. Each year, Planned Parenthood delivers vital sexual and reproductive health care, sex education, and information to millions of people.  

Porchlight strives to reduce homelessness in Dane County by collaborating with the community to provide shelter, affordable housing, and supportive services that eliminate barriers and empower the individuals and families we serve. 

Psychological Addiction Services (PAS) is a private narcotic treatment program accredited by CARF – Commission on Accreditation of Rehabilitation Services. Psychological Addiction Services (PAS) offers counseling, education and resources that encourage healthy choices. Our goal is to assist adults struggling with heroin and opiate abuse. Using medications with counseling services we can help you break free from your addiction. Psychological Addiction Services (PAS) is committed to improving the quality of methadone treatment available to patients includes attempts to reduce stigma associated with medication assisted treatment. Psychological Addiction Services (PAS) has licensed substance abuse counselors and therapists are ready to help you gain control of your life again.

Sugar River Senior Center offers a variety of services, including case management to provide senior adults the support, resources, and community services they need to safely remain in their own homes, healthy and nutritious meals, transportation assistance, and medical equipment loans, such as walkers, canes, wheelchairs.

We specialize in treating people who are addicted to alcohol and/or other drugs. We are unique in that our clients receive both psychological and medical treatment simultaneously by our Dually Certified Medical Team. We also provide stand-alone psychological services, including family and small group counseling. Because every person’s journey is unique, at Tellurian, we meet each of our clients where they are. The UW Health Kids Fitness Clinic helps children and teens between the ages of 5-18 who have concerns about their health, fitness or weight gain. Working with the entire family, we will look at exercise habits, diet and family health history to put together a plan for making healthier lifestyle choices.

For more than 40 years, we at Vivent Health have been relentless in the fight against HIV, from advocating for policy change to protecting communities through prevention efforts to welcoming and caring for all who come through our doors. We offer people living with HIV the highest-quality care, including wraparound social services designed to support the whole person. And perhaps most of all, we provide a safe, judgment-free zone for everyone we serve, regardless of whom they love, the color of their skin, where they live, or how much money they have. All so we can help people living with HIV live long, healthy lives, and work toward ending the HIV epidemic. 

The WIC Program works to improve the health of women, infants and children who may be nutritionally at risk by providing healthy foods, health information, and referrals to health care. 

As part of Safe Kids Worldwide, Safe Kids Wisconsin is one of more than 400 grassroots coalitions in the United States that brings together health and safety experts, educators, corporations, foundations, governments and volunteers to educate and protect families on issues surrounding child safety. Safe Kids Wisconsin supports eleven coalitions that provide safety education, support and programming throughout the state. Safe Kids Wisconsin is led by Children’s Wisconsin, the region's only independent health care system dedicated solely to the health and well-being of children. In addition to providing world-class medical care and research, Children’s Wisconsin works closely with parents, caregivers, educators and communities across the state to offer services and resources that help keep Wisconsin’s kids healthy, happy and safe.

The Green County Adult Drug Court is a program designed to provide individuals the opportunity to address their addictions and move beyond criminal behavior. It is a voluntary program, with entrance occurring pre-adjudication and post-adjudication. The program is abstinence based and intensive in nature. Requirements include mandatory substance use disorder treatment, random drug testing, ongoing appearances before the judge, case management meetings, and attendance of support groups (AA, NA, CA, Smart Recovery, other) or alternative methods that support ongoing recovery.  Participants are also required to obtain employment or pursue educational opportunities, participate in pro-social activities, pay restitution and program fees.
