Purpose: Identify key breakdown points across 9 core life performance areas
Scale: 1–10 (1 = very weak / 10 = strong & consistent)
Time: 10–15 minutes
1. AWARENESS (Clarity & Self-Understanding)
Focus: How clearly the client sees themselves and their situation
Questions:
How clear are you about what is currently not working in your life?
How often do you reflect on your thoughts, emotions, and behaviour patterns?
Do you notice repeated patterns that hold you back?
Scoring:
1–3: Confused / reactive
4–6: Some awareness but inconsistent insight
7–10: High self-awareness and reflection
2. ALIGNMENT (Values & Identity Fit)
Focus: Are they living aligned or misaligned?
Questions:
Do your daily actions match your personal values?
Do you feel like you are living “your life” or someone else’s expectations?
How often do you feel internally conflicted about decisions?
3. ACTION (Execution & Discipline)
Focus: Follow-through and consistency
Questions:
How consistent are you with doing what you say you will do?
Do you struggle with procrastination or avoidance?
How often do you complete tasks that move your life forward?
4. ACCOUNTABILITY (Ownership & Responsibility)
Focus: Ownership mindset vs blame patterns
Questions:
When things go wrong, do you take responsibility or look outward?
Do you have systems or people that hold you accountable?
How often do you justify delaying important actions?
5. AWARE PATTERNS (Emotional & Behaviour Loops)
Focus: unconscious cycles under pressure
Questions:
Do you notice repeating emotional reactions in stressful situations?
Do you tend to shut down, overreact, or avoid under pressure?
How quickly can you regain control when triggered?
6. ABILITY (Skills & Capability Gaps)
Focus: competence vs confidence
Questions:
Do you feel you have the skills needed for your current goals?
Are there specific skills holding you back right now?
Do you know what to learn next, or does it feel unclear?
7. ACTION COMMUNICATION (Expression & Influence)
Focus: communication clarity and impact
Questions:
Do you express your needs clearly to others?
How confident are you in difficult conversations?
Do people often misunderstand you?
8. ABUNDANCE (Money, Opportunity & Value Creation)
Focus: financial flow and opportunity mindset
Questions:
Are you satisfied with your current financial situation?
Do you feel in control of your income growth?
Do opportunities tend to come to you or feel blocked?
9. ACTUALISATION (Purpose & Long-Term Direction)
Focus: meaning, direction, and identity evolution
Questions:
Do you feel you are moving toward a meaningful long-term direction?
If nothing changed, would you still be satisfied 2–3 years from now?
Do you feel you are becoming the person you want to be?
SCORING SYSTEM
For each A, assign score 1–10.
Pattern interpretation:
0–40 total: Crisis / fragmentation → foundational stabilisation needed
41–65 total: Functional but stuck → pattern interruption required
66–85 total: High potential → performance optimisation coaching
86–90 total: High performer → refinement + leadership expansion
COACH INSIGHT LAYER
After scoring, the coach identifies:

2. Secondary resistance pattern
Second lowest → emotional/behavioural reinforcement loop
3. Hidden constraint
Area where client thinks they are fine but are actually misaligned
