Flight Nurse Checklist

Step One

Please send me the following documents to my email Jeffrey.Sprick.2@us.af.mil

• 3 Level AF2096

  • • Must show 3 Level prior to current Fiscal Year (FY23 or older)

• All DD214s from all service periods

• Current Nursing License 

• Formal Training Certificates

Step Two

• Once I receive those documents we work an Incentive Eligibility Verification Worksheet with your Commander.

Step Three

• Upon completion of that form, I will be able to request money against your name.

Step Four

• Once the money is loaded against your name, I can build your contract and have you review and complete it.

Step Five

• After the contract is complete, I build your package and send to NGB for approval.

Step Six

• You get approved.
