What is my child has Asthma and needs an inhaler?

Could you please furnish the Health Office with the following documentation:

1.   Please have your physician complete the attached Massachusetts Asthma Action Plan and have he/she sign at the bottom.  The physician’s signature is absolutely necessary, without the physician’s signature the plan of care is not complete.

2. An Asthma Care Plan and  Medication administration form needs to be filled out by a parent/ guardian   

We will also need the prescribed asthma medication in the pharmacy labeled box to be used at school and a chamber if necessary.

Please make every attempt to have the documentation in over the summer and the medication ready before the first day of school so we can have a safe plan in place for your child.  You may mail documentation over the summer to the address above (attention: School Nurse) or bring documentation and medications to school. Remember that medications cannot travel in student backpacks and must be brought to the health office by a parent.
