Definition of Allowable Exemptions:
There are two situations in which children who are not appropriately immunized may be admitted to school:
Medical Exemption
Documentation from your child's Health Care Provider attesting that the immunization(s) is medically contraindicated must be submitted to the school annually.
Religious Exemption
Written statement by parent or guardian states that the immunization(s) conflict with their sincere religious beliefs must be submitted to school annually.
Philosophical exemptions are not allowed by law in Massachusetts, even if signed by a physician.
Parent/Guardian must notify the nurse annually in writing if their child is exempt from immunizations for medical reasons or sincere religious belief.
