Reporting procedure

Clinicians  are to use the NEW online portal available at: Report a notifiable disease - GOV.UK (www.gov.uk) 

 If this reporting method does not work or is not available please revert to the form version which can be found on All Referral Forms on SystemOne 

It is essential that clinicians ensure that the notification form is completed and submitted to the proper officer within three days or in the event of urgent cases, within 24 hours by telephone.  Where doubt exists, guidance can be sought from:
