** It is BSA and Pack 1776 policy that each member of your family that plans on attending a campout this scouting year must have Medical Forms (Parts A & B) on file with the Pack. Please turn them in at the first Pack Meeting in the Fall.
As the links to the right highlight, the BSA suggests that no unit keep digital records.
The Pack will only accept Medical packets on paper. Please do not send in email.
The Pack saves Medical packets until they expire. At that time, the old forms will be destroyed or return upon request.
The Medical coordinator will keep the records at home in a locked binder, satchel, or backpack.
The Medical coordinator or one of the Key 3 will bring the locked container to Pack campouts.
Here are a few good practices to save every parent time in the long run:
Only provide a copy of the forms to the Pack. It is best that you keep the originals. This is especially important when Part C is required in Boy Scouts!
Complete all but the age, grade, weight, unit leader, and signatures (either digitally or in pen) and save them as a template you can use each year. This way you can easily refresh the forms from year to year by just creating a copy and fill in the items needing changes.
Read the medical forms carefully. All instructions are there.
Fill out everything. Nothing should be skipped.
Sign everywhere it says to... including the section for non-prescription drugs on the second page!
Include dates for all immunizations (write them in or attach the immunizations).
NOTE!! You can print them out from the PISD parent portal!!
Attach a copy of both the front and back of your medical insurance card to each Medical Form packet for each participant.
(Adults) Tetanus shots must be up to date (once every 10 years).
Make note of the talent release on the first page. It can be missed if you rush through.
Finally, please staple all of the documents for each individual.
WARNING! If any of these items are missed, the district and council camps will require you provide them or your child won't be able to participate.
ScoutBook has the ability to track scout and leader medical dates. Navigate to the Scout and then 'Edit Profile'.
Warning: ScoutBook currently does not have a way to export the Medical dates!
Be a stickler with the scout Med forms, making sure that all items are properly filled out according to our notes on the website. This will ensure a smooth registration at the various summer camps. Make sure that these are rectified before or during the first campout.
Adult and Sibling forms are important but given we are at a family campout, I'm not too worried. Let the parents know what was missed and ask that they rectify it after the campout. If they hand them in, I mark the adults that we have in Scout Book (go to the scout and then navigate to each parent).
There is a one time setup for the Medical Report in ScoutBook. However you will have to modify the scout list a few weeks before each campout. In Scoutbook, follow these instructions:
Go to 'My Dashboard'
Reports -> Create New
Cub Scouts Report Type
Custom Scout List
Select all boys from All active Dens (not including Webelos II, Falcon)
Data: Nothing checked
Settings:
Show Health Record Dates
Report Title:
Whatever you choose. I used: 'Pack 1776 Medical'
