1. Eric: Couple of house keeping rules-Fake names, we do not take fake names on charts.
Client came up and said he could give fake names (he called and was told he can give fake names)
Eric told him no, John said to the client over the phone "we do not check ID" after client asked if we could use fake name
Dave: We had an issue with a fake name of a client that tested with a false positive

3. John: Be careful/more secure when disclosing information about building/security (i.e. staying where the keys are in front of the lobby/giving out codes/etc)

30MSM(and TM) complained of blood in semen 2 weeks ago. Few blood cells in urine. Consulted with Dr. George who said to monitor urine and come back to see if red blood cells are still present.
DAVE/CHRIS
35MSMW noticed a bump on side of penis that was fairly large. Hx of rectal warts. Looked at wart, confirmed it, about 3mm. Treated with TCA. Expected that he comes back for further treatment.
21MSW came in with bump/sores. Dr. George said it was just sebaceous cysts. Client had poor STD education. We should have reached out to him better about sexual education.
23MSW came in with warts. Girlfriend told him about it in December, five days later he saw warts coming up. Unsure of who had given it to whom. Treated with TCA and told to come back.
JOBERT - Austin Trainee
36MSM partner told him that he was + for Chlamydia. Urine came back negative and we are awaiting the throat swab. Last partner who was + did not have sex with him for about 90 days as well.
24MSM anal discomfort: bleeding and itchiness when wiping. Another doctor told it was hemorrhoids. Dr. George checked and told to continue using hydrocortisone.
22MSW discharge started yesterday morning. Testicles pulsating when he pees. NGU. Given Doxy. Had two partners recently, one was female three days ago (oral and kind of anal sex), other was with the male roommate of the first partner (non-consensual/sexual assault perhaps).
What is our legal obligation with sexual assault?
Other sections do not have obligation and just refer to counseling. Matching it with them, we assume we are not obligated to report this as well.

41MSW came in reporting that partner had Chlamydia. He was epitreated.
46MSW had one female partner and came in with discharge. Partner had discharge as well. His discharge was yellowish and thick. Found Gram-Positive bacteria that was extracellular. Treated with Ceftriaxone and Doxy.
25MSM rash on arm and thought it was poison oak from hiking. Told to come back if it has not disappeared but did not look like STD. Also confirmed hemorrhoids.
46MSM had yellowish discharge and a history of NSU. Urine sediment only came back with 1WBC and gram stain had wbcs too. Sent out urine for testing.
26MSW had irritation and peeling at head of penis. Looked very inflamed and had been there for 3 years. Dr. George confirmed ballinitis (spelling?) and was given hydrocortisone.
