Entry Form

Please read the following instructions carefully before filling out the form!
Here's how to make your entry REALLY STAND OUT with the judges:
Have your parent/guardian beside you! Entries must be filled out and digitally co-signed by both the student and their parent/guardian TOGETHER.
Check the Rules page one last time to make sure your entry complies with all the requirements!
Please use the first "Email" field to enter the email address of a parent/guardian. We MUST have a non-school email address for all communications about your entry, and for you to receive a copy of your entry form responses. The second field (optional student email address) can be used for a secondary email address, and this could be the student's school or personal email.
Make sure you give your entry a TITLE (try to think of something other than the theme for the title - be original!).
Fill out any special DETAILS about your entry. Please check the rules for your category and division to see what kind of information is needed here, e.g.:
DANCE/FILM: If background music is used, list the title of the song and the artist.
LITERATURE: Provide the word count for the piece.
MUSIC: List the musician(s) and instrument(s).
PHOTOGRAPHY: Provide the dimensions of the piece (in inches/cm/pixels) and information about the camera used.
VISUAL ARTS: Provide the dimensions of the piece (in inches/cm/pixels) and information about the media used.
ACCESSIBLE ARTS DIVISION ONLY: Provide details of the accommodations used in support of the artist's disability when creating this entry. (See the Accessible Arts rules for details.)
Fill out your ARTIST STATEMENT. You have a 100 word limit for this - use as much of it as you can! Remember, interpretation of the theme is worth 40% of the score during judging so really think about what the theme means to you and why it inspired you to create this piece of art. I recommend typing up your artist statement in a separate Google document (hint: use the Google Docs word count, spelling and grammar check tools for that final polish) and then copying and pasting it into your entry form. Please do NOT include your name in your artist statement.
The GRADE DIVISION for entries is Middle School, unless the artist has an IEP or 504 plan and employed special accommodations as described in the Accessible Arts rules, in which case select Accessible Arts Division.
Select the appropriate CATEGORY for your entry.
Upload your file(s). Visual arts entrants - if you're having trouble uploading one or more photos of your entry, you can take a picture using your Chromebook camera and upload that, then either email me the photo(s) separately at acrompton@rbusd.org or bring the entry to me at school so we can take a better photo.
If you are submitting an entry in the VISUAL ARTS category, we would like to display the original entry in our art gallery, so please bring the physical entry to Mrs. Crompton in the library as well as submitting the entry online. PHOTOGRAPHY entries will also be appreciated if you are able to print and mount them yourself, otherwise we will assist with printing the entry.
