Section 4-11-5 - Health Records

Every school must comply with all department of public health requirements. Every school shall have on file a health record for each student enrolled in the school. Upon a student's transfer, student health records are forwarded with the student transcript to the receiving school. Schools shall comply with applicable Family Edu​​​​​cational Rights and Privacy Act rules if they are recipients of certain funds from the U.,S. Department of Education.  Check with the Office of Legal Counsel ​for additional guidance.
