How to Request Health Office Records
Policy Statement:
To maintain confidentiality with student health records in accordance with HIPAA/FERPA requirements and district policy, student health records will not be printed and/or released from the school site. See PUB 648.
Student Records Request by a Parent/Guardian
The parent/guardian obtains a request form from the nurse, SHA, or office staff. It can be found by clicking HERE.
The parent returns the completed form to the school nurse, SHA, or office staff. Staff must verify the parent/guardian's identification and sign off at the bottom of the request.
The nurse, SHA, or office staff will either fax (702-799-2995) or email the request to Risk Management and place the original in school mail, ATTN: Risk Management - SHOMS. To send an email to Risk Management use the Gmail address of SHOMS@NV.CCSD.NET with a subject line of Health Office Request and attach a scanned copy of the signed, verified form. Example:
4. Risk Management will run a report for the requested information and mail directly to the address listed on the request. If necessary, due to an immediate need, Risk Management can either e-mail or fax the health record to the school to be given to the parent.
Student Records Request by a Non-Parent
The requesting party obtains a request form from the nurse, SHA, or office staff. It can be found by clicking HERE.
The requesting party returns the completed form to the school nurse, SHA, or office staff. Staff must verify the requestor's identification and sign off at the bottom of the request.

4. Risk Management will determine what information can be released and will run a report for the requested information. The information will be forwarded directly to the requesting party either by email, school mail, or regular mail.
