Leave of Absence Process - Summary of Leave of Absence Process
Request for Leave of Absence Form - This form is used to request a Leave of Absence. Complete and submit to your supervisor.
Certification of Health Care Provider - Employee - Employee Illness- Use this form to document intermittent or longer illness (i.e. planned surgery, maternity, etc). This form needs to be completed by your physician and returned with Leave Request Form.
Certification of Health Care Provider - Family Member - Family Member - Use this form to document intermittent or longer illness (i.e. planned surgery, maternity, etc) for a family member. This form needs to be completed by the physician.
FMLA Military - Certification of qualifying Exigency for Military Family Leave
FMLA Veteran - Certification for Serious Injury or Illness of a Veteran for Wage and Hour Division Military Caregiver Leave
FMLA Injury Illness Military - Certification for serious injury or illness of covered Service member for Military Family Leave
Return to Work Form - Complete this form and present to your supervisor with documentation from your physician that you are released to return to work (see leave process for details).

Paid Parental Leave
Please review District Rule GBRIC - Employee Pregnancy, Paid Parental Leave, and Unpaid Parental Leave. Paid Parental Leave became effective on June 26, 2023. This benefit allows up to 6 weeks of paid leave for eligible employees.
