You should report:
Security Incidents (Unauthorized access, data breaches, phishing attempts)
Policy Violations (HIPAA, Clean Desk Policy, Workplace Safety, etc.)
Harassment or Discrimination (Any form of misconduct or inappropriate behavior)
Fraud, Waste, or Abuse (Misuse of company resources or unethical financial activities)
Confidentiality Breaches (Unauthorized sharing of sensitive information)

Acknowledgment – You will receive a confirmation of your report.
Review & Investigation – Compliance will assess the situation and initiate an investigation if necessary.
Confidentiality & Protection – Your identity will be protected, and retaliation is strictly prohibited.
Resolution & Feedback – The compliance team will take necessary action and provide updates when applicable.
All reports remain confidential to the extent possible.
ACSI prohibits retaliation against employees who report in good faith.
Anonymous reports will be treated with the same level of importance.
