Dr. Yichun Michelle Fang

Dr. Yichun Michelle Fang, DNP Class of 2020

Dr. Fang is a certified Family Nurse Practitioner. She earned her BSN at New York University, and her MSN and DNP at University of California-Los Angeles. She specializes in primary care and cardiology. Her DNP project evaluated the utilization of an electronic prognostication tool in increasing goals of care conversations with high risk heart failure patients; a quality improvement study to increase documentation of goals of care conversations (GoCCs) for high risk Heart Failure (HF) patients by identifying patients with high (>10%) 1-year mortality via the Gagne Combined Comorbidity score, an electronic prognostication score (E-Gagne). The study evaluated the presence of documented GoCCs by providers, after notification of identified high-risk patients.

The manuscript was published in the Journal of the American Association of Nurse Practitioners. The project was presented at the Quality and Safety Education for Nurses (QSEN) in Practice Conference in 2021.

Dr. Fang is a cardiology nurse practitioner at the Greater Los Angeles VA Medical Center. During her time she has been an advanced practice provider for general cardiology, electrophysiology, pulmonary hypertension, and heart failure. She has developed the Structural Heart program, collaborating with other local facilities to expand services available to veterans. She is actively involved in ongoing cardiology research and ongoing quality improvement projects. She is also a chair of the Nurse Professional Standards Board, championing the onboarding and professional development processes for nurse practitioners.

Dr. Fang also serves as a clinical preceptor for nurse practitioner students and nurse practitioner residents. Her interest in working with the vulnerable population and developing quality improvement projects to improve patient care.
