Hometown Waiver Solutions has over 17 years of experience providing services to individuals with Developmental Disabilities. We serve individuals who are on the Indiana Medicaid Waiver Program.

CIH WAIVER to include:

• Community Transition

• Community-Based Habilitation

• Residential Habilitation and Support

• Respite

• Transportation - Level 1

• Transportation - Level 2

FAMILY SUPPORTS WAIVER to include:

• Community-Based Habilitation

• Participant Assistance and Care

• Respite

• Transportation – FSW

Our services will range from serving as much as 24 hours a day to several hours per week.
