Connections Case Management, LLC

Connections Case Management, LLC is committed to providing appropriate services for children and adults with developmental disabilities. We assist individuals and their families to connect with resources so that they can remain in their homes and communities. We accomplish this through person-centered supports and by identifying the consumer’s strengths and support needs in order to develop an effective plan for services. Connections Case Management, LLC puts the client first in everything we do. We work together to create a culture of inclusion built on trust, respect, and dignity. Connections serves clients of all ages and all disabilities in all of Indiana’s 92 counties.

Connections Case Management also boasts Case Managers who carry fewer caseloads than average. That means they are able to give your case the personalized attention it deserves. We have several seasoned professionals on our case management roster, as well as many parents who have first-hand experience using the Medicaid waiver system for their own children.
