To create a Google Form - via DRIVE (Option 1)

To to Google Drive.

Click + New

Google Forms

Blank Form

You can try using the blank quiz and from a template as well so make sure you check out those options.

Adding a Title and Description

The first step is to give your form a Title and a Description.

Make sure the title reflects what the form is about - this makes it much easier to locate later and also gives the viewer a sense of what is to come.

Once you have named your form click Untitled form in the top left corner and the name will transfer to there.

To create a Google Form - via the Waffle (Option 2)

Click the Waffle to open the Google apps menu.

Scroll down until you see the Forms icon and click on it.

This will open up all of your existing Google Forms, but also give you the option to click the +Blank to create a new form or to use one of the templates from the template gallery.
