1. Get a copy of the Client Satisfaction Survey (CSS) Form in the ERDB Action Officer of the Day. Also available in this link: https://bit.ly/erdbcsm2023
2. Answer the CSM Form.
3. Check the Other Comments/Suggestions portion of the CSM Form.
4. For pen and paper, drop it in the designated drop box in the Action Officer of the Day or email at erdbrecords@gmail.com
Everyday, the Action Officer of the Day shall open the drop box, forwards to AFMD Management and Audit Section (MAS). MAS shall compile and record all feedback received.
Feedback requiring answers shall be forwarded to the relevant offices where they are required to answer within three (3) days upon receipt of the feedback.
The answer of the concerned office shall then be relayed to the client.

Make sure to provide the following information:
a. Name of person(s) being complained
b. Incident/transaction
c. Evidence
Everyday, the Action Officer of the Day opens the dropbox; AFMD Management and Audit shall handle all complaints received online.
Received complaints shall be forwarded to the Office of the Director for initial assessment. Letter complaints received by the Director likewise undergoes initial assessment, then forwards to concerned division/RDECs being complained.
The Division/RDEC concerned investigates the complaint, prepares report and letter reply; Forwards to Director for signature.
Records Unit sends reply to the complainant through fax, post mail or electronic mail.
All complaints must be acted upon within 72 hours from receipt.
