Homebound

General Information:

• The decision for a student to receive homebound services is an IEP team decision but also requires physician/psychiatrist completion of the Request for Hospital-Home Instruction Form. If consensus is lacking, the school team should speak with the doctor to assess the basis for the recommendation.

  • The Request for Hospital-Home Instruction Form is only good for one school year. For students receiving homebound instruction from year to year, a new request must be submitted to Student Services at the start of each school year. 

  • Homebound services are meant to be temporary in nature and therefore must be reviewed by the IEP team every 30 school days. 

• Homebound services are for students who are physically or mentally unable to attend school for at least 10 consecutive school days.  

• Homebound services may be appropriate in order for students to continue to receive special ed services due to disciplinary action.

• CMCSS requires a Special Populations Coordinator and a homebound teacher from Student Services to be invited to all IEP meetings where homebound services will be proposed or considered.
