Homebound
General Information:
The decision for a student to receive homebound services is an IEP team decision but also requires physician/psychiatrist completion of the Request for Hospital-Home Instruction Form. If consensus is lacking, the school team should speak with the doctor to assess the basis for the recommendation.
The Request for Hospital-Home Instruction Form is only good for one school year. For students receiving homebound instruction from year to year, a new request must be submitted to Student Services at the start of each school year.
Homebound services are meant to be temporary in nature and therefore must be reviewed by the IEP team every 30 school days.
Homebound services are for students who are physically or mentally unable to attend school for at least 10 consecutive school days.
Homebound services may be appropriate in order for students to continue to receive special ed services due to disciplinary action.
CMCSS requires a Special Populations Coordinator and a homebound teacher from Student Services to be invited to all IEP meetings where homebound services will be proposed or considered.
Homebound Example:
Services should be based upon student need, but we must offer a minimum of 3 hours per week in order to generate the appropriate Option (funding) in TN Pulse.
The location should always be "home" even if the student is receiving homebound services at Greenwood.
