End of the Year Procedures

The checklist below was created to help ensure each Special Education Case Manager has a successful close to each school year. Special Education Case Managers should complete the form by the designated date for their school's Special Education Lead Teacher to review prior to the last day of school.

For the End of Year (EOY) procedures in 2022-2023, CMCSS SPops Department moved to Google Forms to collect EOY forms. You can view the printable version of the EOY checklist below; however, you will be expected to electronically submit your EOY checklist via Google Forms at the end of the school year.
