Plan of Action to Address Objectives!
Objective 1.1
Monitor students’ attendance to analyze the grades of those who have been in the program for 30 days or more.
Create a spreadsheet showing beginning and ending dates to focus only on active students
No transition for enrichment; Students will stay with the same teacher for academics and enrichment.
This will maximize academic time and give teachers more time to build/strengthen relationships with their students.
Case Management will incorporate teachers intentionally monitoring grades bi-weekly
Comparison between Q2 averages and their progress throughout Q3
Parent Engagement Opportunities:
Parents are invited to come in for homework help
We will provide a designated space where parents can work with their child regarding homework as well as classwork
Teachers will be available to help parents navigate Infinite Campus as well as Google Classroom
Incentives in February, March, and April for students who are showing growth based on Q2 averages and Q3 progress
Incentives will include free time during enrichment, treat bags, and/or local fieldtrips
Continue the partnership with Level Up’s free tutoring program
Objective 2.1
Monthly incentives for student attendance
Students will be recognized and awarded for not missing more than 2 days in one month. Awards can include field trips, goodie bags, or free enrichment days.
Afterschool attendance data and Day school data will be assessed monthly.
Objective 2:2
Compare Fall and Spring semesters' behavior reports from the Day school data. If students showed growth from semester 1 to semester 2, that will count toward improvement
Growth is based on a decrease in the number of referrals from semester to semester.
PBIS student assessments to address their understanding of the importance of positive behavior using their PBIS standards :
Students will assess their behavioral improvement using a survey.
Provide incentives for students with no disciplinary infractions.
Awards can include field trips, goodie bags, or free enrichment days.
Recommendations
a. An integral component of evaluating the success of GCSU’s OHMS YES Program lies in a review of program data. The review of Cayens shows faulty data due to input eras, incorrect registration of adults/ caregivers, and missing data. It is recommended that the OHMS YES Program establish a process for the reconciliation of data to ensure accuracy and fidelity. High data fidelity enables informed decision-making, improves program efficiency, and fosters trust in data-driven decisions. Inaccurate or missing data can result in unreliable outcomes and could ultimately affect 21st CCLC Program funding. Discuss ways to ensure data accuracy and establish a schedule for consistent data review by multiple parties.
b. As noted in the FY24 Summative Evaluation, the Annual Scavenger Hunt event is quite an accomplishment, especially with the culmination of the event being the Family Fun Day! It is recommended the YES Program administrators document this event as one of your Parent Events in Cayens and document parent participation to demonstrate the buy-in and support that the YES OHMS Program has from the community at large and parents/ families of participating youth! Although this event is currently documented, attendance is not included which negatively impacts the program’s ability to meet Objective 3.1 and Objective 3.2.
Plan to Action to Address Recommendations!
The Coordinator will assign a teacher at the site to assist with data review. The Director will review monthly for accurancy and send confirmation emails that verify the review was completed.
The Coordinator attended a training on 9.30.25 with the BCSD info systems coordinator and the external evaluator to discuss the correct methods to categorize data in Cayen.
