Supporting your child through transition

Primary to Secondary Transition involves:

• General/SEND information requested from primary schools.

• Primary transition meetings with Year 6 teachers and/or SENCo.

• Transition sessions with primary school pupils run by secondary subject teachers.

• Enhanced transition for identified pupils from primary schools.

• Year 6 Induction Days.

• Review meetings e.g. Annual Reviews/TAFs.

Post-16 Transition:

• Liaison with Careers and Participation Services in Year 9.

• Careers education Years 9-11, including information about post-16 options.

• Further meetings with Careers and Participation Services in Year 11.

• Information about sixth forms, colleges and training providers.

• Transition meetings are arranged and relevant information is shared with support staff at the college or training provider.

• In addition we have our school's Careers Advisor who is available to speak with all students and parents on request.
