The Mobile Integrated Community Healthcare (MICH) program has positively progressed with our partners from the Department of Aging and Disabilities to assist the citizens who are in need of assistance. Receiving nearly 250 referrals for clients needing assistance.  We have worked to reduce avoidable 911 calls, emergency room visits, and ultimately have reduced admissions to our county hospitals.  We do this by connecting our clients with various resources and empowering them to better manage their healthcare needs and improve their quality of life. 

In addition to working with our clients, in the past year, we have presented at the National Association of Mobile Integrated Healthcare Providers (NAMIHP) annual conference in Charlotte, NC.  We continue to look for enhancements to better our program.
