We understand that at times, a student may need to be excused from all or part of an activity due to illness or injury. To excuse your child from activity, please see the procedures below:
1) For excuses 1-3 days in length:
Students must have a handwritten note from their parent guardian that includes the following:
The Date(s) that the student will need to be excused
The reason for the excuse
Parent/guardian signature
Specific modifications to activity (if applicable) that will allow partial participation (i.e. no jumping, biking ok, etc)
2) For excuses in 4+ days in length:
any student needing to be excused from activity 4 or more days, is REQUIRED to have note from a physician on file. The note must include the following:
The Date(s) that he student will need to be excused (including an anticipate return date)
The reason for the excuse
Physician signature
Specific modifications to activity (if applicable) that will allow partial participation (i.e. no jumping, biking ok, etc)
Students should be prepared to dress out for class even on days they are excused from activity. In many cases, students can continue to participate in modified activities with permission from their physican. If possible, please ask your doctor to allow "Modified PE" so that we can keep your child active in class until cleared for full participation.
Students without a note, or who have a note indicating "NO PE" will not be permitted to participate in any activity until cleared.
