Medication Permission

Please print and complete yearly if your child requires daily medications, or as needed medications, that will be stored and administered during the school day. Your child should bring all medicine to the nurse's office upon arrival at school. If your child is to self-carry medications, please see Medication Permission: SELF-CARRY.
