Please print and complete yearly if your child requires daily medications, or as needed medications, that will be kept in the child's backpack, or locker, and self-administered during the school day. We do not allow controlled medicines, such as ADHD medicines to be self-carried. Narcotics are not allowed to be given at school and your child should not attend school if narcotics are needed for pain control. This is for safety purposes and also learning is altered when taking narcotics. Thank you for your help! Kristin, RN
